RN Care Coordinator – Flexible Hours, Home Care
2nd Family Home Care is seeking a Registered Nurse (RN) Care Coordinator to support our non-medical home care clients and caregivers. This role serves as the primary clinical point of contact for clients and families, ensuring care plans are followed, reassessed, and properly coordinated.
This position is ideal for an RN who enjoys care coordination, case management, and working directly with caregivers and families, all while maintaining a flexible schedule.
Position Details
• Location: Colorado
• Specialty: Non-Medical Home Care
• Reports To: Director of Nursing or Owner
Compensation
• $100 per case (covers ongoing case management)
• $50 per in-person assessment
• Start with 3–5 cases, with opportunity to grow your caseload
• Paid bi-weekly
Schedule
• Flexible scheduling based on caseload
• In-home assessments required every 90 days for Colorado clients
• Combination of remote coordination and occasional in-home visits
Job Summary
The RN Care Coordinator oversees the clinical coordination of home care services to ensure clients receive safe, consistent, and high-quality support. Responsibilities include developing care plans, supervising caregivers, conducting nursing reassessments, and maintaining clear communication with families and staff.
Responsibilities
• Serve as the primary RN contact for clients and their families
• Communicate with clients and families via phone, email, and Family Portal
• Develop, maintain, and update individualized care plans
• Coordinate client schedules with the scheduling team
• Communicate care plan details to caregivers prior to their first shift
• Conduct 90-day nursing reassessments and care plan reviews
• Perform and document in-home client assessments
• Supervise CNA/GNA caregivers as required
• Observe and document client symptoms, changes, and progress
• Maintain accurate documentation in ADLware
• Report changes in client condition to appropriate HIPAA representatives
• Ensure compliance with agency policies and infection control standards
• Participate in quality improvement initiatives
• Assist with caregiver orientation and ongoing training as needed
Qualifications
• Active Registered Nurse (RN) license
• Minimum 2 years of nursing experience
• Experience in home care, case management, or care coordination preferred
• Experience conducting in-home assessments in a home care setting is an advantage
• Strong communication and interpersonal skills
• Excellent documentation and organizational skills
• Ability to work collaboratively with clients, families, caregivers, and agency staff
