About the position
Responsibilities
• Providing appropriate and effective services leading to successful linkage and adherence to healthcare.
• Assisting and collaborating in conducting and developing comprehensive assessments and service plans.
• Responsible for activities relating to healthcare retention.
• Providing assistance in accessing primary medical care and other necessary services for clients.
• Providing crisis intervention to clients as needed.
• Conducting new client orientation to their neighborhood, including locating stores, hospitals, and community services.
• Learning case management principles and practices and about community resources.
• Performing multiple assignments simultaneously.
Requirements
• Strong communication skills, including active listening and verbal/written proficiency in English.
• Ability to work effectively with individuals living with HIV or at risk for seroconversion, and those facing homelessness and substance abuse issues.
• Ability to tactfully interact with jail staff, clients, community agencies, and the public.
• Organized, responsible, and efficient in task management.
Nice-to-haves
• Familiarity with the criminal justice system, either through personal experience or working in a jail setting.
• Experience in recovery from drug and/or alcohol use disorder, or familiarity through friends/family.
• Knowledge of HIV and PrEP services in the greater Atlanta area.
