Key Responsibilities
 Conduct home visits and perform comprehensive social work assessments, counseling, case management, and crisis intervention.
 Co-facilitate support groups, offering a safe space for patients and their caregivers.
 Organize and coordinate monthly educational programs to empower and inform stakeholders.
 Engage in chapter-wide events to foster community connection and outreach.
Required Skills
 Active New York State LSW license.
 Minimum of three years of relevant experience in social work, with an emphasis on end-of-life care.
 Exceptional communication and interpersonal skills.
 Ability to work autonomously as well as collaboratively within a team environment.
 Bilingual capabilities are advantageous.
 Valid driverÂs license and access to a vehicle.
Qualifications
 Licensed Social Worker (LSW) with a valid New York State license.
 At least three years of direct experience in a similar capacity.
 Demonstrated knowledge and experience in end-of-life care settings.
