Job Responsibilities:
Provide telephonic or virtual nursing care and education to patients.
Assist with care coordination, case management, and follow-up services.
Collaborate with multidisciplinary teams to deliver high-quality care.
Document patient interactions accurately and maintain compliance with company policies.
Support CVS Health initiatives, including chronic care management and wellness programs.
Qualifications:
Active Registered Nurse (RN) license in your state of residence.
Minimum of 2 years of clinical nursing experience (telehealth experience preferred).
Strong communication, problem-solving, and critical thinking skills.
Proficiency in using digital tools and electronic medical records.
Ability to work independently and maintain high productivity in a remote setting.
