Citizens Memorial Home Health Services is recruiting a Social Worker to provide counseling to patients for behavioral change. CMH is made up of 34 primary and specialty clinics, 6 long-term care facilities and 1 residential care facility. About the department: The Medical Home Services Department provides patients with a physician-led care team. The program emphasizes coordination, communication and evidence-based care, resulting in an improved patient experience and outcome. A typical day will include: Working with a set of clinics, providers, and nurse care managers to coordinate and provide integrated care to treat the physical, mental, spiritual, and social needs of our patients. • Helps patients identify barriers and behaviors that lead to poor outcomes • Identifies goals and develops strategies or behavioral modifications to achieve goals • Assesses patients in the Emergency Department • Provides transitional care services by making referrals for patients moving from the ED and Primary Care into therapy This position is: Full-time, Mon-Fri, 8am-4:30pm; remote work option available for candidates living within the 8-county service area Job Requirements: • Education: MSW or Master's in Counseling from an accredited institution • Experience: 1 year of experience preferred • Skills: Written and oral communication skills, time management, detail-oriented • Licensure/Certification: LMSW, LCSW, or LPC required • Physical: Sitting for prolonged periods of time; ability to lift up to 20 lbs. on an infrequent basis Benefits Include: • Competitive salary • Annual merit increase • No-cost health insurance option for employee • Full suite of insurance benefits available for employee and family • Early access to pay with on-demand PayActiv Program • Employee assistance program • Increased matched savings to Retirement Plan • PTO • Company-paid long-term disability plan • Employee discounts Connect with us!
