They provide a centralized intake and referral partner service function to gather pertinent information to optimize referral partner satisfaction, patient satisfaction, and quality care. The Intake Coordinator plays a key role in the agency’s success, often sitting at the nexus of patients, clinicians, referral sources, and leadership. Responsibilities: Supports external business development team members throughout the intake process by preparing the patient’s medical record with a high level of accuracy and ensuring a timely admission in accordance with physician orders. Monitors EMR systems / Careport for new referrals and electronic faxes for all other pertinent information regarding patient information for an initial visit. Acts as the main point of contact for accounts not actively targeted to regulate incoming referrals and coordinate the start of care across the referral partner, business development team, and clinical team as needed. Perform eligibility/insurance verification. Qualifications: Active Licensed Practical Nurse Licensure. Ability to set objectives and handle multiple tasks, clients, and projects simultaneously in a fast-paced environment; ability to appropriately balance priorities, deadlines, and deliverables. Ability to be flexible when needed, take initiative, and demonstrate accountability. Ability to handle confidential protected health information and material. High-quality customer service and phone skills are essential. Basic Microsoft Office proficiency is required. High level of organizational skills and strong attention to detail.
