Remote Social Worker - Continuing Health Care Opportunity

We are seeking a dedicated and experienced Social Worker to work remotely with our discharge to assess team, promoting independence and well-being for older adults and adults with complex needs. This 6-month contract offers a competitive hourly rate of £32, hybrid working arrangements, and a comprehensive benefits package.
This exciting opportunity requires a skilled and compassionate Social Worker with experience in completing Care Assessments, knowledge of Continuing Healthcare, and excellent communication skills. As a key member of our team, you will work collaboratively with partner agencies, conduct face-to-face assessments, and develop personalized care packages to meet the unique needs of each individual.
Key Responsibilities:
Conduct comprehensive Care Assessments and develop personalized care plans
Collaborate with partner agencies to promote independence and well-being
Conduct face-to-face assessments in individuals' own homes or care settings
Attend monthly meetings in Basingstoke and participate in joint assessments as required
Monitor, review, and reassess care packages to ensure effectiveness and achieve desired outcomes
