Remote RPM Coordinator - LPN
Overview
Chronic Care Staffing is seeking a Remote RPM Coordinator to support patients in managing chronic and high-risk conditions through Remote Patient Monitoring (RPM). You will play a key role in monitoring biometric data, educating patients on device usage, escalating critical findings, and coordinating care with providers to improve patient outcomes and reduce hospitalizations.

Employment Type: Full-Time
Pay Range: $19.5 ā $24/hour
Responsibilities
ā¢ Conduct onboarding and device education calls for new RPM patients.
ā¢ Monitor incoming biometric data (blood pressure, glucose, weight, oxygen saturation) daily.
ā¢ Identify abnormal readings/trends and escalate per protocol.
ā¢ Perform monthly engagement calls to assess patient condition and address concerns.
ā¢ Troubleshoot device connectivity and usage issues.
ā¢ Communicate effectively with patients and caregivers regarding goals and progress.
ā¢ Promote adherence to individualized care plans.
ā¢ Accurately document interactions in EHR/RPM platforms.
ā¢ Educate patients on lifestyle changes and condition management.
ā¢ Comply with CMS RPM billing and documentation guidelines.
ā¢ Maintain productivity expectations for monitoring and outreach.
Requirements
Must Have Criteria
ā¢ Unencumbered active CMA/RMA certificate OR compact LPN/RN license.
ā¢ Active BLS certification.
ā¢ Strong knowledge of CCM/RPM protocols and billing requirements.
ā¢ Proficiency with EHR systems and Google Suite.
ā¢ Recent experience in a clinical or CCM setting.
ā¢ Ability to work independently in a HIPAA-compliant home office.
ā¢ Strong problem-solving, documentation, and communication skills.
ā¢ Ability to meet call volume and productivity expectations.
ā¢ Nice to Have
ā¢ Prior experience in care coordination, chronic care, transitional care, or remote patient monitoring.
ā¢ Familiarity with community resources and patient/family education.
ā¢ Background in quality measures, reporting, or CMS compliance.
ā¢ Demonstrated teamwork in a remote environment.
ā¢ Additional certifications/coursework in care management or case management.
