Responsibilities
Conduct telephone and virtual assessments to understand members' medical needs.
Develop and implement individualized care plans in collaboration with members.
Proactively address challenges and identify opportunities to improve health outcomes.
Coordinate services and programs to support members' overall health and well-being.
Ensure compliance with regulatory requirements and quality standards.
Qualifications
Required: 3+ years of experience with Virginia Medicaid waivers, 3+ years of oversight care insights, 2+ years of clinical practice experience, and current, unrestricted Virginia RN license.
Preferred: Experience with Medicaid waivers.
