Position Overview

Key Responsibilities

Deliver care management services using telephonic and electronic systems.
Ensure compliance with all organizational policies and regulatory standards.
Qualifications
Licensure: Current and unrestricted RN license.
Experience: Minimum of 3 years of clinical experience, preferably in care management, case management, or home health.
Knowledge: Strong understanding of chronic disease management and care coordination principles.
Skills: Proficiency with electronic medical record (EMR) systems; exceptional organizational and communication skills.
Independence: Ability to work autonomously while managing multiple priorities effectively.
