As an Autism Services Navigator, you'll play a pivotal role in assessing, planning, and coordinating care for TRICARE beneficiaries enrolled in the Autism Care Demonstration (ACD) program. As an Autism Services Navigator, you'll be an integral part of this program, working closely with families, healthcare providers, and other stakeholders to ensure that individuals with ASD receive the best possible care. Key Responsibilities: Leading Care Coordination and Advocacy Efforts As a Remote RN Autism Service Navigator, you'll be responsible for: Collaborating with families to assess their needs and develop individualized Comprehensive Care Plans (CCPs) that ensure access to quality, cost-effective care. Monitoring and updating CCPs to reflect changing needs and circumstances, ensuring seamless care coordination. Advocating for families by addressing barriers to services, locating specialized resources, and facilitating timely enrollment decisions. Providing education on ACD and benefits, empowering families to make informed decisions about their care. Evaluating case management outcomes to ensure beneficiary satisfaction and compliance with care plans, driving continuous improvement. Participating in medical team conferences and coordinating with civilian and military healthcare providers to ensure comprehensive care. Essential Qualifications: Bringing Expertise and Compassion to Autism Care To excel as a Remote RN Autism Service Navigator, you'll need: A current unrestricted license as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW). Certification in Case Management (CCM) or the ability to obtain certification within four years of hire. A minimum of three years of direct clinical care experience with a focus on pediatrics, behavioral health, or autism. Familiarity with HIPAA regulations and case management best practices. Strong organizational, problem-solving, and communication skills, with the ability to navigate complex care systems.
