Key Responsibilities Review and analyze incoming patient data, including vital signs, symptoms, and other relevant health indicators, to identify any critical or out-of-range readings. Promptly address any critical or out-of-range readings and escalate as necessary according to established protocols. Monitor patients' vital signs and health data remotely using specialized monitoring equipment and software. Communicate with patients via phone to discuss their health status, provide guidance, and answer questions. Explain medical terminology and procedures in an understandable manner, ensuring patients are informed and engaged in their healthcare management. Address patient concerns and questions promptly, providing reassurance and guidance where necessary. Manage a significant volume of patient calls within established timelines. Work towards achieving personal and team performance metrics. Foster a collaborative and positive work environment. Maintain the Nsight culture of positivity. Participate in cross-training and other LVN duties as assigned. Requirements and Qualifications To be successful in this role, you must have: An active Compact LVN/LPN license.
