Job Description: Care Partner (RN, LPN, or LVN) Location: Remote (NY & NJ-based patients) We're looking for a compassionate, detail-oriented clinician to join our team as a Care Partner. In this role, you’ll serve as a key touchpoint for patients recently discharged from New York hospitals after a stroke or brain injury. Using the PRISM platform, you’ll hold virtual check-ins, monitor health data, and coordinate care to ensure patients stay on track in their recovery. This role is ideal for someone who thrives on patient connection, loves solving problems, and is excited about the possibilities of remote-first healthcare. Over time, this position may transition to fully remote depending on your preferences.
