Key Responsibilities
Ā Manage patient care and attend necessary medical appointments.
Ā Coordinate hospital visits, procure medical equipment, arrange home modifications, and facilitate additional supportive services.
Ā Operate independently, maintaining a flexible schedule while traveling locally as required.
Ā Deliver outstanding care and support to patients, ensuring optimal treatment outcomes.
Ā Engage in travel for in-person visits, which will comprise 50%-75% of your time.
Required Skills
Ā Active Oregon RN License.
Ā Valid Oregon Driver's License along with Auto Insurance.
Ā Proficiency in Microsoft Word and Excel.
Ā Bilingual proficiency in Spanish is advantageous.
Ā Strong ability to work autonomously while also functioning as part of a collaborative team.
Qualifications
Ā A minimum of 1 year of Nursing Experience (preferred).
Ā A minimum of 1 year of Case Management Experience (preferred).
Ā Active RN License is required.
Ā Openness to travel locally for in-person visits (50%-75%).
