As a Remote Nurse Care Specialist for CVS Health, you will be responsible for providing telephonic support to patients with chronic conditions. You will assess patient needs, provide education and support, and collaborate with healthcare providers to develop care plans. Requirements:
1. Active RN license
2. Minimum of 2 years of clinical experience
3. Strong computer skills
4. Ability to work independently and within a team
5. Excellent communication and interpersonal skills
6. Knowledge of chronic disease management
7. Ability to work in a fast-paced environment
Responsibilities:
1. Conduct telephonic assessments of patient needs
2. Develop and implement care plans in collaboration with healthcare providers
3. Provide education and support to patients on managing their chronic conditions
4. Document patient interactions in electronic health record system
5. Coordinate care with other healthcare providers as needed
6. Follow up with patients to monitor progress and adjust care plans as necessary
7. Participate in team meetings and continuing education opportunities
