About the Role As our ideal candidate, you'll be a self-starter with a strong background in home health care coordination, excellent communication skills, and the ability to work independently in a fast-paced environment. This role offers the flexibility of telecommuting, allowing you to work from the comfort of your own space while maintaining a high level of productivity and engagement. Key Responsibilities Coordinate physical and occupational therapy staff schedules to match agency needs with the right therapist. Manage a high volume of patient charts (150-200 weekly), ensuring timely updates and follow-ups. Address incoming emails and phone calls regarding patient care and staffing, providing prompt and effective solutions. Communicate effectively with therapists regarding patient visit schedules and documentation. Review patient charts to identify missed visits and notify clinicians for updates. Interpret admin comments on patient charts and determine the proper course of action. Perform other duties as assigned by the Administrator, demonstrating flexibility and a willingness to adapt. Maintain a high level of organization, typing quickly and accurately (at least 45-50 WPM). Essential Qualifications To succeed in this role, you'll need: A strong foundation in home health care coordination, with experience in managing patient charts and coordinating with therapists. Proficiency in Microsoft Office, particularly Word and Excel. Excellent communication and interpersonal skills, with the ability to interact with patients, therapists, and administrators. A detail-oriented and problem-solving mindset, with the ability to multitask and manage multiple priorities. A reliable and stable internet connection, computer, and noise-cancelling headphones. Ability to work independently without constant supervision. A positive and enthusiastic attitude, with a commitment to delivering exceptional patient care. Preferred Qualifications While not mandatory, the following qualifications can give you a competitive edge: Leadership or management experience, demonstrating your potential for growth and career advancement. Spanish speaking skills, allowing you to communicate effectively with a broader patient base. Familiarity with medical terminology, enhancing your ability to navigate patient charts and communicate with clinicians.
