Remote Medical Assistant/LPN for Virtual Patient Care (Bilingual EN/SP Preferred)

Lara Health is seeking a compassionate and experienced Medical Assistant (CMA) / Licensed Practical Nurse (LPN/LVN) to join our Chronic Care Management (CCM) / Remote Patient Monitoring (RPM) team. This is a fully remote role with flexible scheduling, ideal for someone passionate about chronic care and patient engagement.
Position Highlights
ā¢ Schedule: Minimum 25 hours/week
ā¢ Location: Fully remote (U.S. based only)
ā¢ Experience Required: Minimum 5 years caring for patients with chronic conditions (e.g., diabetes and HTN, cardiology, primary care)
Role Responsibilities
ā¢ Monitor patient vitals (blood pressure, blood sugar, etc.) and medication adherence
ā¢ Support patients in scheduling MD visits and making lifestyle changes
ā¢ Maintain accurate and up-to-date EMRs
ā¢ Collaborate with providers and care team
ā¢ Perform other duties as assigned
Required Skills & Qualifications
ā¢ Licensure: Active MA/LPN license (mandatory)
ā¢ Experience: 5+ years in clinical setting, preferably in chronic care settings
ā¢ Language: Bilingual English/Spanish strongly preferred
ā¢ Strong communication, organizational, and time management skills
ā¢ Ability to work independently and collaboratively
ā¢ Proficiency with computers and EMR systems (must have own device and reliable internet)
ā¢ Familiarity with medical terminology and clerical tasks
ā¢ Passion for working with older adults and delivering preventative care
Preferred Experience
ā¢ Familiarity with remote care coordination tools and EMR-based task management
ā¢ Ability to delegate, prioritize, and oversee multiple patient cases simultaneously
