Job Summary We are seeking a licensed practical nurse (LPN) to join our remote care team. In this role, you will provide ongoing support, care coordination, and patient education to chronically ill patients via phone and digital health platforms. If you have a passion for patient advocacy, chronic disease management, and telehealth, this position is perfect for you.
