Remote LPN Care Manager - Hybrid Role (Clay, Coosa, Randolph & Talladega Counties, AL)
We are seeking a compassionate and detail-oriented LPN Care Manager to join our team in a hybrid-remote role, requiring occasional on-site work in our office setting. As a Care Manager, you will play a vital role in ensuring seamless transitions across the health services spectrum for our patients, facilitating wellness and recovery through person-centered care plans.
In this position, you will work collaboratively with clinical and non-clinical staff, external providers, and community resources to assist clients in overcoming barriers to care, improving health outcomes, and promoting overall well-being. Your strong communication and problem-solving skills will enable you to effectively navigate complex patient needs and develop integrated care plans that address physical, behavioral, and social health requirements.
Your key responsibilities will include:
Assessing and re-evaluating patient needs to determine care requirements and preferences
Creating and implementing personalized care plans in conjunction with patients and caregivers
Identifying and addressing barriers to care, including insurance coverage issues and social determinants of health
Coordinating patient care activities, sharing information among treatment team members, and providing education on disease management and healthy living skills
Advocating on behalf of patients, tracking progress, and providing feedback to the treatment team
Maintaining accurate and timely documentation of care coordination in electronic health records
To succeed in this role, you should possess:
An Alabama Licensed Practical Nurse (LPN) license in good standing
At least three years of experience in nursing, with a preference for experience working with special populations
Proficiency in managing multiple priorities, accurate documentation, and electronic health records
Demonstrated ability to work collaboratively with multi-disciplinary teams and access community resources
Strong communication, problem-solving, and interpersonal skills
