We are seeking an empathetic and experienced Licensed Telehealth Therapist to become an integral part of our dedicated Care Team. In this role, you will be instrumental in delivering tech-enabled psychotherapeutic care, ensuring the well-being of our members through a holistic integration of social and clinical services. While your primary focus will be on providing care through telehealth, opportunities to engage with members in their homes and communities may arise as necessary.

Key Responsibilities
• Assess and diagnose chronic and acute behavioral health conditions.
• Collaborate with a multidisciplinary health team to create and implement individualized treatment plans.
• Integrate a behavioral health perspective into comprehensive care planning for complex cases.
• Conduct initial assessments, identify risks, provide therapeutic interventions (individual, family, and group), and link members to community resources for enhanced care access.
• Work alongside primary care providers, psychiatrists, and other professionals to discuss treatment plans and monitor progress.
• Deliver psychoeducation to members, focusing on mental health promotion and disease prevention.
• Facilitate referrals for members needing additional specialty behavioral health care and provide guidance on treatment plans until engagement in appropriate care occurs.
• Build and maintain trusting relationships with patients, aligning care with their goals and values.
• Participate in multi-disciplinary case conferences to ensure comprehensive and holistic care.
• Offer coaching and education to team members on supporting patients with behavioral health challenges.
• Assist with post-discharge planning to help prevent readmissions.
• Accurately document member care and coordination in our electronic medical record (EMR) system.
• Contribute to the development of clinical pathways and protocols to continuously enhance patient care.

Required Skills
• 2+ years of outpatient experience working with adolescents and adults enrolled in Medicaid, Medicare, or dual-eligible.
• 3+ years of experience in assessing, planning, and managing behavioral patient care through direct clinical work.
• Familiarity with trauma-informed and evidence-based psychotherapy treatments.
• Competence in making timely and precise clinical assessments in challenging situations with composure.
• Strong collaboration skills, effectively partnering with patients, primary care providers, and mental health specialists.
• Proficient in electronic medical record documentation and leveraging technology to improve care.

Qualifications
• Active license as an LCSW, LMFT, or LCPC; MFT and CPC interns may also be considered.
• Minimum of 1 year of telemedicine experience.
• Experience working with underserved populations facing socioeconomic barriers to healthcare.
