Remote Licensed Clinical Therapist (LCSW/LMFT) - Make a Difference in Healthcare

In this role, you will provide direct assessment, diagnosis formulation, treatment planning, treatment intervention, and psycho-education to patients via a telehealth platform. You will also have the opportunity to coordinate care with other community services and agencies, and provide behavioral health/crisis intervention support to patients in primary care clinics. As a key member of our team, you will work collaboratively with the Program Director and Patient Services Team to ensure the quality of our behavioral health program.

Requirements:
Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) in good standing in the State of California
Master's Degree in Social Work (MSW) or Master's in Marriage and Family Therapy from an accredited school
Minimum three (3) years of clinical experience in a medical group, affiliated model, hospital, or medical office/clinic setting
Strong commitment to the mission, policies, goals, and philosophy of Planned Parenthood, including support for the full range of reproductive options
Preferred Qualifications:
Certification in Case Management (CCM, ACM)
Experience working in a telehealth and/or primary care environment
Three (3) years' experience in case management
Bilingual (reading, writing, speaking) English/Spanish or English/Vietnamese
