The Home Health Occupational Therapist will provide exceptional OT services to patients in their homes, focusing on functional improvement and quality of life enhancement. Responsibilities Functional Assessment: Conduct comprehensive evaluations of patients' ability to perform activities of daily living (ADLs), instrumental ADLs, and meaningful occupations in their home environment Treatment Planning: Develop individualized intervention plans with measurable goals focused on maximizing independence in self-care, home management, and community participation Therapeutic Interventions: Implement evidence-based occupational therapy techniques including ADL training, cognitive rehabilitation, upper extremity therapy, and adaptive equipment training Home Modification Recommendations: Assess home environments and recommend adaptive equipment and modifications to enhance safety and independence Patient & Caregiver Education: Provide expert guidance on energy conservation techniques, joint protection strategies, and compensatory techniques for daily activities Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments Care Coordination: Collaborate effectively with physicians, nurses, other therapists, and healthcare team members to ensure comprehensive patient care Progress Monitoring: Regularly reassess patient progress, adjust treatment plans as needed, and ensure adherence to clinical standards and regulatory requirements Skills Graduate of an accredited Occupational Therapy program (MOT, OTD, or equivalent) Valid Illinois Occupational Therapist license NBCOT certification (or eligibility) Current CPR certification Valid driver's license and auto insurance Open to new graduates and experienced occupational therapists - comprehensive training provided Willingness to learn WellSky Kinnser EMR system (training included) Strong clinical assessment and critical thinking abilities Outstanding written and verbal communication skills Patient-centered care philosophy with cultural sensitivity Strong organizational and time management capabilities Passion for community service and home-based healthcare Authorization to work in the United States or eligibility for H1B sponsorship English proficiency equivalent to successful therapy practice in the U.S Commitment to long-term employment with Safe Life Home Health Care Prior home health experience Benefits Premium health, dental, and vision insurance coverage Flexible spending accounts (FSA/HSA) Life and disability insurance options Comprehensive home health training program for all new hires Extensive tuition reimbursement program Continuing education opportunities and clinical mentorship Career advancement pathways within our growing organization Professional certification support and EMR training Flexible scheduling options to accommodate personal needs Paid time off and holiday benefits Home office stipend for remote documentation H1B visa sponsorship for qualified international candidates Employment-based green card application assistance Immigration legal support and guidance throughout the process Company Overview Safe Life Home Health Care provides post-surgical care, pain management, medication administration, and home infusion nursing services.
