Key Responsibilities • Provide clinical support to nursing homes by assisting MDS Coordinators with resident assessments in line with state and facility regulations. • Conduct thorough reviews of clinical records to ensure accuracy and consistency in documentation practices. • Mentor and train MDS Coordinators on assessment protocols and documentation standards. • Engage hands-on with facility-specific needs to facilitate optimal care. • Identify issues within facilities and work collaboratively to create and execute effective action plans. • Perform interventions, audits, and follow-ups based on the unique requirements of each facility. Required Skills • Active Registered Nurse (RN) license in Pennsylvania. • Minimum of 1 year experience as an RN Assessment Coordinator with a strong understanding of the MDS process. • Demonstrated ability to mentor and train nursing staff. • Proficient clinical assessment skills. • Willingness to travel between various locations as needed. Qualifications • A valid Registered Nurse (RN) license in Pennsylvania is required. • A minimum of 1 year of experience in MDS coordination is necessary. • Flexibility to travel as needed to multiple client facilities.
