Remote Field Care Coordinator, RN - Southwest or Southeast New Mexico

As a Field Care Coordinator, RN, you'll play a vital role in coordinating care for members with complex medical and behavioral needs, promoting improved health outcomes and enhanced quality of life.
Key Responsibilities:
Conduct comprehensive needs assessments to identify medical, behavioral, and socioeconomic needs
Develop and implement personalized care plans, leveraging evidence-based guidelines and collaborative partnerships with internal care teams, providers, and community resources
Engage members through face-to-face and telephonic interactions, providing education, coaching, and support to promote self-management and lifestyle changes
Advocate for members and families, ensuring their needs and choices are fully represented and supported by the healthcare team
Utilize MS Office, including Word, Excel, and Outlook, to document and manage care plans, and maintain accurate records
Requirements:
Current, unrestricted independent licensure as a Registered Nurse
3+ years of clinical experience, with 2+ years of relevant healthcare experience
1+ year of experience with MS Office, including Word, Excel, and Outlook
Reliable transportation and ability to travel within assigned territory (up to 50% of the time)
Residence in Southwest or Southeast New Mexico, with flexibility to work remotely
Preferred Qualifications:
Master's degree or higher in a clinical field
Commission for Case Manager (CCM) certification
3+ years of clinical experience as a Registered Nurse, with 1+ year of community case management experience
Background in Managed Care
