Role Responsibilities
Conducting medication reviews to enhance patient safety and treatment effectiveness.
Processing acute medication requests and making informed clinical decisions.
Reviewing hospital discharge summaries to ensure accurate medication reconciliation.
Assisting GP practices in achieving QOF and IIF targets as needed.
Contributing to the management of long-term conditions within the multidisciplinary team.
Delivering medicines optimisation and structured medication reviews.
Requirements
Must be an Independent Prescriber
GPhC registration
Strong communication skills
Self motivated
Experience working in GP practices
