Responsibilities
As a Remote Clinical Case Manager, you will be a vital point of contact for patients, families, providers, and other stakeholders. You will:
Coordinate patient care across the continuum of care, including referrals, home visits, and community resources.
Assess patient needs and develop care plans to improve health outcomes and ensure access to benefits.
Communicate with patients and partners to provide education, support, and advocacy.
Maintain accurate and timely documentation of case management activities.
Collaborate with a multidisciplinary team to optimize patient well-being.
Ensure compliance with all applicable regulations and guidelines.
Proactively address issues to improve patient outcomes and enhance the overall patient experience.
Qualifications
Required:
Demonstrated ability to effectively communicate and build rapport with individuals and families.
Strong organizational and time management skills.
Ability to work independently and as part of a team in a remote environment.
Reliable internet access and a dedicated workspace.
Preferred:
Experience in clinical case management, care coordination, or home medical services.
Bilingual proficiency in Spanish/English.
Experience with Federal health insurance populations.
Valid driver's license and reliable transportation for occasional in-person visits within a designated geographic area.
