Remote Case Manager Registered Nurse - Transforming Healthcare through Compassionate Care

As a remote Case Manager RN, you will play a vital role in assessing, planning, implementing, and coordinating comprehensive case management activities to promote overall health and well-being. With a focus on delivering compassionate care, you will evaluate members' medical needs, facilitate services, and collaborate with interdisciplinary teams to ensure continuity of care.

In this remote role, you will enjoy a Monday-Friday schedule (8:00 AM-5:00 PM EST) with no nights, weekends, or holidays required. As a Case Manager RN, your key responsibilities will include:


Conducting comprehensive assessments of members' physical, emotional, and social needs
Developing and implementing individualized care plans based on clinical findings and member goals
Coordinating services across healthcare providers, specialists, and community resources
Monitoring and evaluating care plans regularly to improve outcomes
Advocating for timely and appropriate healthcare services to support member well-being
Documenting case activities, interventions, and communications accurately and in compliance with regulatory requirements
Collaborating with interdisciplinary teams to ensure continuity of care
Participating in team meetings, training sessions, and quality improvement initiatives
Traveling locally for in-person case management activities as directed by leadership or business needs


To be successful in this role, you will need:


An active state license as a Registered Nurse in good standing, valid in the region where duties are performed
Compact RN License or license in multiple states, or willingness to obtain within 3-6 months
3-5 years of clinical practice experience
2-3 years of experience in case management, discharge planning, or home health care coordination
Comfort working remotely and independently using collaboration tools and virtual communication platforms
Willingness and ability to travel within a designated geographic area for in-person case management activities as needed


Preferred qualifications include:


Bilingual proficiency in Arabic, Korean, or Spanish
Compact RN license or licensure in multiple states
Strong analytical and problem-solving skills
Excellent communication, organizational, and interpersonal skills
Self-motivated and able to work independently
Proficient in navigating multiple systems and applications; skilled in typing and keyboard use
Familiarity with corporate software tools such as Microsoft Word, Excel, Outlook, PowerPoint, and proprietary applications


We offer a competitive pay range of $54,095.00 - $116,760.00, depending on experience, education, geography, and other relevant factors.
