Remote Case Manager – Concierge Regenerative Medicine (Medical Assistant or Nursing Background)

Peak Health is a fast-growing concierge regenerative medicine company providing in-home and telehealth-based care. We are seeking a detail-oriented, compassionate Case Manager with a background in medical assisting, nursing, or patient coordination to manage patient care and treatment journeys. This is a remote position with flexible hours and opportunity for growth.

Key Responsibilities:
• Coordinate care and scheduling with patients post-consultation
• Support patients via phone/text with treatment logistics and education
• Ensure proper documentation in EHR and CRM systems
• Track follow-up touchpoints and patient progress
• Liaise with nurse practitioners, sales, and logistics teams

Preferred Qualifications:
• Background as MA, CNA, LPN, RN, or similar
• Excellent phone and communication skills
• Tech-savvy: comfortable with CRMs, EHRs, telehealth platforms
• Experience in concierge, stem cell, or regenerative medicine a plus
• Bilingual (Spanish) is a plus

Compensation: Competitive pay, performance-based bonuses, and growth opportunities.

Job Type: Full-time

Pay: $15.00 - $20.00 per hour

Expected hours: 40 per week
