As a Case Management Specialist, you will be responsible for assessing member needs, communicating with Primary Care Providers, maintaining updated care plans, and participating in Interdisciplinary team meetings.
Key Responsibilities:
Conduct assessments and create individualized care plans to address member needs and goals
Collaborate with the Care Team to improve health outcomes and address social determinants of health
Communicate effectively with members, Primary Care Providers, and community-based organizations
Maintain accurate and up-to-date records, including care plans and member interactions
Participate in Interdisciplinary team meetings to discuss member care and outcomes
Requirements:
3+ years of experience in a health-related field
Bachelor's Degree or equivalent experience
Strong organizational and communication skills
Ability to work independently and maintain professional relationships
Proficient in Microsoft Office Software and computer systems
Preferred Qualifications:
Medicare and Medicaid experience
CRC, CDMS, CRRN, COHN, or CCM certification
Managed Care experience
Experience working with geriatric special needs, behavioral health, and disabled populations
Bilingual (English/Spanish; English/Creole)
