Key Responsibilities
Conduct thorough assessments to identify clinical and psychosocial needs of individuals.
Address health and safety concerns, including social determinants of health.
Collaborate with a multidisciplinary team to create a personalized Life Plan.
Regularly monitor and update Life Plans to ensure optimal care.
Facilitate access to medical and community resources to promote health and wellness.
Coordinate referrals for clinical, long-term, and social support services.
Engage families and natural support systems to enhance care coordination.
Ensure that services provided are culturally and linguistically appropriate.
Advocate for individuals to foster self-advocacy and independence.
Utilize Health Information Technology for efficient documentation and communication.
Maintain compliance with HIPAA, Medicaid, and all relevant regulatory standards.
Keep precise and detailed records of services rendered.
Participate in ongoing training and professional development to enhance skills.
Required Skills
Proven experience working with individuals with intellectual and developmental disabilities.
Strong organizational and time management capabilities.
Ability to operate independently and effectively manage crisis situations.
Basic proficiency in technology for documentation and communication.
Valid drivers license.
Qualifications
A Bachelor's degree coupled with a minimum of two years of relevant experience, OR
A Registered Nurse (RN) license with at least two years of pertinent experience (not restricted to case management/service coordination), OR
A Master's degree with one year of relevant experience.
