Conducting initial health assessments, including condition-specific issues and medications
Documenting clinical history and activities of daily living
Evaluating mental health status, cultural and linguistic needs, and caregiver resources
Collaborating with healthcare professionals, patients, and families to ensure seamless care coordination
To be successful in this role, you'll need:
An Associate's Degree in Nursing
An active RN license
Strong communication and assessment skills
Ability to work independently in a remote setting
