SUMMARY Performs triage functions for after hours program. Receives calls, plans for care interventions, dispatches on call nurses or provides phone support as needed, notifies appropriate nurse, team or outside agency of any changes. ESSENTIAL DUTIES AND RESPONSIBILITIES 1. Mentors on-call staff as appropriate. 2. Performs triage role utilizing nursing judgment and process to provide effective, timely and compassionate care to patients. 3. Dispatches on-call nurses as needed and makes home visits if dictated by workload or geographical distance. 4. Provides telephone and telehealth support to family at time of death. 5. Serves as clinical and procedural resource for on-call staff. 6. Answers calls within reasonable time. a. Hourly monitors and responds to messages and requests. 7. Reviews triage documentation to assure accurate and timely notes. 8. Attends meetings as requested. 9. Communicates with physicians and other care providers as needed. 10. Ensures adherence to policies. 11. Notifies on-call administrator of problems. 12. Other duties may also be assigned. SUPERVISORY RESPONSIBILITIES No Supervisory Responsibilities. CONTINUING EDUCATION The agency requires this position to complete 15 hours of continuing education per year covering topics that will contribute to improvements in carrying out the above responsibilities. Regulatory agencies may require some disciplines to have additional hours in order to be licensed or certified.
