Compensation:
$25 – $40 per hour (RN)
$20 – $30 per hour (LPN)
(Based on experience, certifications, and program expertise)
About the Role:
We are hiring experienced Massachusetts-licensed RNs and LPNs to support patients in Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Behavioral Health Integration (BHI), and Collaborative Care Model (CoCM) programs.
This is a fully remote role focused on delivering high-quality, compliant care management services to Medicare and chronic care patients.
You will play a critical role in improving outcomes, reducing hospitalizations, and ensuring adherence to CMS care management guidelines.
Key Responsibilities:
Chronic Care Management (CCM):
• Deliver monthly care coordination (20+ minutes per patient)
• Develop, update, and manage individualized care plans
• Perform medication reconciliation and adherence monitoring
• Coordinate with PCPs, specialists, and caregivers
• Maintain accurate, CMS-compliant documentation
Remote Patient Monitoring (RPM):
• Monitor daily patient vitals (BP, glucose, weight, SpO2, etc.)
• Identify abnormal trends and triage/escalate clinically when needed
• Educate patients on device use and adherence
• Ensure RPM compliance (16+ days data/month)
• Support billing compliance (CPT 99453, 99454, 99457, 99458)
Behavioral Health Integration (BHI) / CoCM:
• Conduct behavioral health screenings (PHQ-9, GAD-7, etc.)
• Support care plans for patients with depression, anxiety, and other behavioral conditions
• Collaborate with psychiatric consultants and primary care providers
• Track patient progress and adjust care interventions
• Document time-based services aligned with CMS billing requirements
Qualifications:
Required:
• Active RN or LPN license in Massachusetts
• Hands-on experience with CCM and/or RPM programs
• Strong understanding of CMS documentation and billing requirements
• Experience with behavioral health workflows (BHI or CoCM preferred)
• Proficiency with EMR/EHR systems
• Strong patient communication and engagement skills
Preferred:
• Certification: CCM (Certified Case Manager) or Behavioral Health certifications
• Experience with Medicare / Medicare Advantage populations
• Familiarity with platforms like Athenahealth, eClinicalWorks, Epic
• Bilingual (Spanish) is a plus
