Registered Nurse (RN) – IV Infusion & Wound Care
About the Role
My Heart Care Services is a private-pay home care and home health agency providing concierge-style in-home IV infusion and wound care services. We are seeking an experienced Registered Nurse (RN) to deliver skilled nursing care, provide clinical oversight, and supervise LVN-delivered services in the home setting.
This role is ideal for RNs who want flexibility, clinical autonomy, per-visit compensation, and leadership responsibility—without Medicare, OASIS, or CMS paperwork.
Key ResponsibilitiesClinical Care (IV & Wound Services)
• Perform skilled nursing visits including IV infusion therapy and wound care
• Initiate and manage peripheral IVs and infusion protocols
• Conduct comprehensive wound assessments and treatment planning
• Monitor patient response and clinical progress
• Identify and manage complications or adverse reactions
• Provide patient and family education related to treatment and recovery
Clinical Oversight & Supervision
• Develop and approve nursing plans of care
• Delegate and supervise LVN services in accordance with Texas BON rules
• Provide clinical guidance and escalation support to LVNs
• Review LVN documentation and visit notes
• Serve as clinical escalation point for patient condition changes
Care Coordination
• Communicate with physicians or ordering providers as needed
• Coordinate care transitions (post-hospital or post-procedure)
• Collaborate with agency leadership on care delivery and quality
• Support continuity of care across multiple visits
Documentation & Quality
• Complete accurate and timely clinical documentation
• Maintain compliance with agency policies and nursing standards
• Participate in quality assurance and case reviews
• Support infection control, safety, and incident reporting processes
Compensation
• Per-Visit Pay (rates vary by service type and complexity)
• Mileage Reimbursement for travel between patient visits
• Additional compensation may apply for:
• Same-day / urgent visits
• Weekend or after-hours visits
• High-complexity or extended visits
Schedule & Work Environment
• Flexible PRN or part-time scheduling
• Field-based role providing care in client homes
• No routine on-call requirements unless agreed separately
Qualifications
• Active, unrestricted Texas RN license
• Minimum 2 years of clinical nursing experience preferred
• IV infusion and wound care experience strongly preferred
• Experience supervising LVNs preferred
• Strong clinical judgment and documentation skills
• Reliable transportation and valid Texas driver’s license
• Current CPR certification
• TB screening and background check required
Preferred Certifications
• IV Therapy Certification
• Wound Care Certification (WCC, CWCA, or equivalent)
• Experience in home care, infusion therapy, or private-pay nursing
