About the positionResponsibilities
Develop and maintain one-on-one relationships with patients.
Teach patients how to manage their health issues effectively.
Celebrate patient successes in healing and recovery.
Conduct patient home visits primarily during weekdays.
Complete OASIS documentation with significant support.
Work independently while managing patient care.
Requirements
Minimum 1 year of acute-care nursing experience.
Registered nurse license in good standing.
Current CPR certification.
Current TB test and Fingerprint Card.
Current AZ driver's license and auto insurance.
Reliable vehicle for driving to client locations.
Ability to work independently.
Nice-to-haves
Experience in home health care settings.
Strong communication and interpersonal skills.
Ability to adapt to changing patient needs.
