Requirements
Active and unrestricted RN license in the state of residence.
Minimum of 2 years of clinical nursing experience.
Experience with telehealth and remote patient monitoring is preferred.
Strong communication and interpersonal skills.
Proficiency with electronic health records (EHR) systems.
Ability to work independently and manage time effectively.
Reliable internet connection and a private, quiet workspace.
Responsibilities
Assess and evaluate patient health conditions, symptoms, and needs through telehealth platforms.
Provide patient education on disease management, medications, and lifestyle modifications.
Develop and implement personalized care plans in collaboration with the healthcare team.
Monitor patient progress and adjust care plans as necessary.
Maintain accurate and up-to-date patient records in the EHR system.
Coordinate with other healthcare providers and specialists as needed to ensure comprehensive patient care.

Associate or Bachelors degree in Nursing from an accredited institution.
Current, active RN license in good standing.
