Key Responsibilities:
Conduct telephonic or video-based assessments of patients
Create and maintain personalized care plans

Educate patients and families on medication adherence and preventive care
Document patient interactions in compliance with HIPAA and internal guidelines

Qualifications:
Active and unencumbered RN license in the United States
Associate or Bachelor of Science in Nursing (BSN preferred)
Minimum 2 years of clinical nursing experience (telehealth or case management a plus)
Proficient with telehealth tools and electronic medical records (EMRs)
Excellent communication and critical thinking skills
Reliable internet connection and a quiet, HIPAA-compliant work environment
Ability to work independently and as part of a remote team
