We are looking for a Field Registered Nurse or our Arizona team. Join our team and make a positive impact on the lives of others! Position Overview: The objective of this role is to assess the medical eligibility of individuals applying for the Arizona Long Term Care Services (ALTCS) program. It's a unique opportunity to help build out processes that will directly impact the lives of patients across AZ and work for the team dedicated to bringing affordable long-term care to as many people as possible. The ideal candidate will have: 1) In-depth knowledge of ALTCS medical eligibility criteria 2) Experience with ALTCS PAS assessments 3) Understanding of Medicaid/ALTCS rules and clinical documentation Requirements: Valid AZ Nursing License- LPN 2+ years of experience working with elderly population Has a patient centered apporach to patient education Direct Experience with ALTCS Eligibility (PAS) assessments Why work at FreedomCare?
