Registered Nurse, Home Health - Full Time Opportunity in Chicago, IL

In this rewarding role, you will provide admission, case management, and follow-up skilled nursing visits for home health patients, administer on-going care and case management, and confer with physicians to develop personalized plans of treatment. You will also have the opportunity to work closely with other healthcare professionals, including Physical Therapists, Occupational Therapists, and Home Health Aides, to ensure comprehensive and coordinated care.
Responsibilities:
Provide admission, case management, and follow-up skilled nursing visits for home health patients
Administer on-going care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager
Confer with physicians to develop personalized plans of treatment based on initial patient assessments and physician orders
Provide hands-on care, management, and evaluation of the care plan, teaching patients and their families in accordance with physician orders
Revise care plans in consultation with physicians based on ongoing assessments and as required by policy/regulation
Coordinate care with other healthcare professionals, including Physical Therapists, Occupational Therapists, and Home Health Aides
Report patient care, condition, and progress to physicians and Clinical Managers on a continuous basis
Implement patient care plans in conjunction with patients and their families to assist them in achieving optimal resolution of needs and problems
Coordinate, oversee, and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions and care plans
Monitor the completion of documentation by home health aides and personal care workers as part of supervisory and leadership responsibilities
Discharge patients after consultation with physicians and Clinical Managers, preparing and completing necessary clinical documentation
Prepare medical documentation, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes
Requirements:
Diploma, Associate, or Bachelor's Degree in Nursing
Minimum of one year of nursing experience preferred, with strong med-surg, ICU, ER, or acute experience
Home Health experience is a plus
Current and unrestricted Registered Nurse licensure
Current CPR certification
Strong organizational and communication skills
Valid driver's license, auto insurance, and reliable transportation required
