Registered Nurse for Senior Home Care, case management w/ Flexible schedule

Perform routine scheduled visits with clients to monitor care and oversee CNAs and caregivers in the home. Maintain detailed and relevant care plans. Assist Clinical and Marketing Teams when your expertise is needed.

Responsibilities
Conduct Weekly and Bi-Weekly visits to Clients
In person home visits as needed within 2hr time period
New clients assessments in a timely manor
Maintain accurate and current care plans
Coordinate Client needs with their PCP as needed
Provide counsel and guidance to family members on managing conditions
Requirements:
Active RN License in Virginia or a compact state
Driver's License and vehicle
Current CPR
One year in home care experience preferred
