Registered Nurse Care Ambassador

About the position

The RN Care Ambassador is responsible for planning, organizing, directing, and coordinating health and home care services for clients. This role aims to improve client outcomes, promote continuity of care, and ensure effective communication among interdisciplinary teams, patients, and families. The RN will oversee daily clinical operations in home care, striving to enhance the quality of life for clients and their families.

Responsibilities
Ā• Complete comprehensive in-person intake assessments, gathering medical history and client preferences.

Ā• Resolve inquiries posed by clients and families to ensure quality of care.

Ā• Complete clinical documentation and intake summary reports within established time frames.
,
Ā• Communicate effectively with clinical and non-clinical staff, clients, and families.
,
Ā• Conduct weekly client visits to reassess care and guide home care staff.
,
Ā• Utilize problem-solving and decision-making approaches to identify and address issues.
,
Ā• Receive case referrals and review patient information to determine home care needs.
,
Ā• Assign appropriate clinicians to cases and create individualized care plans.
,
Ā• Perform after-hours and weekend on-call responsibilities for clients and families.
,
Ā• Provide ongoing communication with family, primary care physicians, and care teams.
,
Ā• Act as a liaison between clients, families, and social systems to collaborate on care.
,
Ā• Ensure compliance with rules, regulations, and departmental procedures.
,
Ā• Complete clinical quality tracker audits to meet regulatory requirements.

Requirements
Ā• 2+ years of experience in inpatient, case management, home health, or a relevant field.

Ā• Bachelor's degree in nursing.
,
Ā• Valid CA Registered Nurse license.
,
Ā• Strong assessment and clinical skills.
,
Ā• Experience with the geriatric population preferred.
,
Ā• Willingness to travel within LA county, especially in West LA.
,
Ā• Familiarity with dementia and chronic diseases common to the aging population.
,
Ā• Experience in customer service in high-stress situations.
,
Ā• Proven ability to create and maintain processes and procedures.
,
Ā• Outstanding leadership, decision-making, and managerial skills.
,
Ā• Proficient with Microsoft Office Suite and Google Office tools.
,
Ā• Strong interpersonal skills and ability to resolve conflict.
,
Ā• Excellent verbal and written communication skills.
,
Ā• Self-starter with demonstrated initiative and ability to multi-task.
