Department: Outpatient Pharmacy Shift/schedule: Full Time (Salaried) GENERAL SUMMARY The Supervisor will oversee and coordinate Medication Therapy Management (MTM) services, Population Health initiatives, and remote pharmacy operations across the health system and its affiliated community pharmacies. This role supports clinical program development, ensures quality performance outcomes, and fosters collaboration between pharmacists, providers, and other healthcare professionals to improve patient health, optimize medication use, and reduce overall healthcare costs. They also ensure high-quality, efficient, and compliant delivery of remote pharmacy services, including managing the pharmacy call center, remote prescription data entry, remote patient counseling, and remote prescription verification (PV1) functions, with a focus on optimizing workflow, maintaining accuracy, ensuring regulatory compliance, and supporting patient-centered care in a virtual service environment. QUALIFICATIONS Education: • High School Diploma or successful completion of an equivalent High School Exam Required • Bachelor's degree in Pharmacy (5 year) is required • Doctor of Pharmacy (Pharm D) is also preferred Licensure: • Pharmacy (OH or KY) license, depending on state of practice, required. • Graduate Pharmacists may also be hired awaiting license • BLS within 90 days of hire required • Completion of PGY1 or PGY2 residency in ambulatory pharmacy care is preferred Experience: • Minimum of 3 years of pharmacist experience, with at least 1 year in MTM, ambulatory care, or population health program management. • Supervisory or team leadership experience is preferred. • Experience with payer-based MTM platforms, health system EHRs, and clinical documentation. • Experience in remote pharmacy operations, central fill, call center management, or PV1 preferred. JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time. 1. Assist the Community Pharmacy Manager in managing day-to-day operations of MTM, population health programs, and remote pharmacy services. 2. Provide guidance, coaching, and performance feedback to pharmacists, pharmacy technicians, and other clinical staff engaged in MTM, Population Health, remote pharmacy services, and related initiatives. 3. Complete performance reviews for all staff currently under their leadership. 4. Act as a liaison between clinical pharmacy services, community pharmacies, and other health system departments. 5. Act as a central liaison between remote pharmacy teams, community pharmacies, and other health system departments. 6. Oversee scheduling and resource allocation to ensure adequate staffing for MTM and population health services. 7. Lead implementation and monitoring of MTM programs, including Comprehensive Medication Reviews (CMRs), Targeted Medication Reviews (TMRs), Targeted Intervention Program (TIPs), and chronic disease management. 8. Oversee development and execution of population health strategies, including medication adherence interventions, preventive care measures, and high-risk patient outreach. 9. Monitor and evaluate program outcomes, reporting metrics to leadership and recommending improvements. 10. Support integration of MTM services into the health system’s care model and electronic health record systems. 11. Work with physicians, nurses, care coordinators, and population health teams to align MTM services with broader health initiatives. 12. Develop and maintain partnerships with community pharmacies to ensure consistent delivery of high-quality MTM services. 13. Engage with patients, caregivers, and community organizations to promote awareness of MTM and preventive health services. 14. Ensure all MTM and population health activities comply with state and federal regulations, payer requirements, and accreditation standards. 15. Maintain accurate documentation of all patient interactions, interventions, and outcomes. 16. Prepare reports and presentations for internal stakeholders and external partners. 17. Ensure accuracy, efficiency, and compliance in remote prescription data entry and PV1 verification. 18. Oversee remote patient counseling, ensuring patient understanding, adherence, and safety. 19. Monitor call center performance, ensuring timely response to patient and provider inquiries. 20. Collaborate with onsite pharmacy teams to streamline workflow between remote and physical pharmacy operations. 21. Develop and implement quality control processes for data entry, counseling, and PV1 services. 22. Ensure all remote services comply with federal, state, and local regulations, as well as payer and accreditation requirements. 23. Monitor performance metrics and generate operational and quality reports for leadership review. 24. Participate in continuous improvement initiatives to enhance service quality and efficiency. 25. Maintain and troubleshoot remote pharmacy technology, including secure communications, order processing systems, and verification platforms. 26. Ensure adherence to HIPAA and other privacy standards for all remote operations. 27. Optimize workflows to minimize turnaround time without compromising quality or safety. 28. Coordinate with prescribers, nurses, and other healthcare providers to resolve medication issues identified during remote processing or counseling. 29. Communicate regularly with community pharmacy teams to ensure seamless integration of remote services. 30. Provide training and onboarding for new remote pharmacy staff. 31. Participates in pharmacy leadership development and competencies, including key pharmacy meetings and pharmacy retreats. 32. Compounds and dispenses medications and pharmaceutical supplies according to accepted standards of practice to fill written prescriptions issued by qualified prescribers. 33. Checks the accuracy and appropriateness of all medication dispensed. 34. Intervenes clinically with the physician when necessary and appropriate to optimize drug therapy. 35. Follows accepted standards of practice and adheres to federal, state, and local regulations, as well as the policies and procedures of SOMC and Pharmacy Services. 36. Serves as a drug information source for patients, physicians, nursing personnel, hospital staff, visitors and the community. 37. Assists upper management with certain assigned reports or projects. 38. Maintains a level of professional competency and remains current in the field of pharmacy. 39.
