This position ensures clear and appropriate visit utilization using evidence-based and best practice guidelines to achieve positive clinical outcomes and efficient use of resources. In addition, this position identifies and collaborates with clinical supervisors on feedback and coaching opportunities for clinical colleagues. Additionally, this role works under the supervision of the Department Manager. Responsibilities for this role include overseeing case management activities and ensuring adherence to best practices. This role contributes to the overall efficiency and effectiveness of case management processes. The value you'll bring to the team: • Evaluates visits per episode for optimal utilization, ensuring the best clinical outcomes and efficient use of resources. • Analyzes utilization to ensure visits align with episode guidelines and best practices. • Develops and revises utilization policies based on past practices to improve efficiency. • Collaborates with colleagues and field clinicians to achieve strategic aims, top decile outcomes, and patient satisfaction. • Mentors colleagues and encourages the implementation of best practice guidelines. • Determines patient risk for rehospitalization and appropriate care pathways.
