This role is responsible for reviewing and ensuring the accuracy, completeness, and consistency of OASIS assessments and ICDâ10 coding, while collaborating closely with clinical and leadership teams to improve outcomes and agency performance.This position is ideal for a detailâoriented Home Health RN with strong knowledge of OASIS, Medicare regulations, quality assurance, and clinical documentation review.Key ResponsibilitiesProspectively review all OASIS assessments to ensure accuracy, completeness, and compliance with federal and state home health regulations and Sequoia Home Health standardsAnalyze OASIS variation and alert reports to identify documentation gaps and trendsEnsure correct ICDâ10 coding and sequencing, including primary diagnoses and coâmorbiditiesCollaborate with home health clinicians to clarify documentation issues and support appropriate correctionsReview visit utilization to ensure services align with patient condition, care guidelines, and reimbursement requirementsIdentify potential underutilization, overutilization, or financial risk and report findings to Clinical Supervisor or designeeCommunicate problematic trends in OASIS data, outcomes, and processes to leadershipParticipate in Quality Improvement (QI), performance improvement, and Corporate Compliance activitiesAssist with additional chart audits and clinical documentation reviews as neededQualifications & RequirementsRegistered Nurse (RN) with an active California license2+ years of home health nursing experience requiredStrong working knowledge of OASIS (OASISâE preferred) and ICDâ10 home health codingInâdepth understanding of CMS regulations, Medicare Conditions of Participation (CoPs), and state licensure requirementsStrong analytical skills with attention to detail and excellent clinical judgmentEffective communication and collaboration skills with interdisciplinary teamsThe employer for this position is stated in the job posting.
