Position : Project Manager with Care Case Management Migration Location : Remote client is in Hartford, CT Duration : Contract role. Candidate must be in around Connecticut. Key Responsibilities • Lead endtoend application and data migration workstreams for care/case management systems. • Manage requirements gathering, gap analysis, data mapping, ETL oversight, testing strategy, and cutover planning. • Ensure PHI protection and HIPAA compliance throughout migration and testing phases. • Coordinate with case managers, families, support providers, IT teams, vendors, and senior leadership to build consensus and alignment. • Create and maintain project schedules, dashboards, KPIs, status reports, meeting agendas, and executive briefings. • Design and execute change management activities including communication plans, training design, and facilitation. • Operate within PMO governance and report program status to leadership. • Negotiate scope, budget, and schedule with vendors and stakeholders to resolve conflicts and keep program on track.
