Job Description:
We are looking for a reliable LVN/LPN (Licensed Practical Nurse/Licensed Vocational) to administer direct patient care, administer approved medications, perform treatments, make accurate nursing observation, and chart on the patients' medical record in a professional and considerate manner. Duties are performed under the direction of the Case Manager and/or Patient Care Manager. LVN/LPN professionals must be first and foremost qualified and compassionate. The ideal candidate preference will have at least one year experience as a nurse in a health care setting. Home health experience preferred. We offer competitive pay and flexible schedule.

Educational Requirements:
• graduate of an approved program of vocational/practical nursing
Licensing/Certification/Registration Requirements:
• current Licensed Vocational/Practical Nurse (LVN/LPN) from the Texas Vocational Nurse Examiners
• current CPR
• BLS certification preferred.

• professional in appearance and practices good personal hygiene

Physical Requirements:
• able to see/hear/speak sufficiently to participate in written/verbal communication.
• able to deal effectively with stress.
• able to use fine motor skills for patient assessment.
• able to stand, sit, bend, stoop, ambulate uneven surfaces, and lift up to 50 pounds.
• able to operate a motor vehicle and travel to prospective patient’s residences.

Responsibilities:
• assists in assessing patient and family deeds through the recognition and reporting of signs and symptoms and nursing observation to the assigned Case Manager
• assists in assessing the health status of assigned patients.
• complies with the established nursing assessment guidelines/policies.
• utilizes the written plan of care for implementing nursing interventions necessary to accomplish the established goals.
• complies with general performance standards of licensed vocational nurse. Demonstrates flexibility in response to unexpected changes in work volume, emergencies, staffing, or scheduling changes.
• must have a reliable means of transportation, proof of financial responsibility (car insurance and a valid state driver’s license)
