Develop and implement population health programs and initiatives to improve the health and well-being of Aetna members and communities. Collaborate with cross-functional teams to develop and execute population health strategies aligned with Aetna's overall mission and goals. Utilize data analytics and healthcare technologies to identify and address health disparities and gaps in care for Aetna members. Continuously monitor and evaluate the effectiveness of population health programs and make necessary adjustments to ensure positive outcomes. Stay up-to-date on industry trends and best practices in population health management and incorporate them into Aetna's strategies and initiatives. Act as a subject matter expert on population health, providing guidance and support to other teams within Aetna. Foster relationships with key stakeholders, including healthcare providers, community organizations, and government agencies, to promote collaboration and drive successful population health initiatives. Develop and manage budgets for population health programs and initiatives, ensuring resources are allocated effectively to achieve desired outcomes. Maintain a deep understanding of Aetna's member population and their unique healthcare needs, and use this knowledge to inform population health strategies and initiatives. Act as a champion for population health within Aetna, promoting awareness and understanding of its importance and impact on member outcomes. Lead and mentor a team of population health professionals, providing guidance, support, and professional development opportunities. Monitor and comply with all regulatory and compliance requirements related to population health initiatives and ensure Aetna's adherence to industry standards. Participate in conferences, seminars, and other professional development opportunities to continuously enhance knowledge and skills related to population health management. Collaborate with marketing and communication teams to develop and execute communication strategies to promote population health initiatives and drive member engagement. Represent Aetna in external meetings and events related to population health, building relationships and promoting Aetna's commitment to improving healthcare outcomes.
