Physician House Calls Program – New York City, Long Island, Northern Westchester, Staten Island, Bronx, and Brooklyn Northwell Health’s Department of Medicine is seeking a BC/BE Internal Medicine or Family Medicine Physicians to join our House Calls Program. Addressing the needs of today’s chronically ill and aging population, Northwell’s House Calls Program provides expert medical care within the privacy and comfort of patients’ homes. The House Calls Program serves patients in New York City, Long Island, Brooklyn, Staten Island, Bronx, and Northern Westchester, who have difficulty traveling to doctor appointments, and looking to expand to Staten Island and Brooklyn. Our care team models consist of physicians, nurse practitioners, nurses, social workers, and medical coordinators that provide telemedicine and at-home visits to diagnose, treat, and coordinate the highest quality care at home. In-home diagnostics include comprehensive labs drawn by phlebotomists, x-rays, ultrasounds, echocardiograms, and more. This practice combines personal connection with patients and caregivers with high quality medical care that is tailored to meet the needs of the individual patient. Our award-winning program was chosen by the Center for Medicare & Medicaid Innovation (CMMI) to participate in the Independence at Home (IAH) national demonstration of the effectiveness of home-based primary care. Northwell’s House Calls programs was recently ranked number one nationally for performance in (IAH) by Medicare. The safety of our patients and our staff are of the utmost importance. Our enrollment process ensures a firm understanding of social situation as well as the medical situation prior to home visits. The appropriate PPE will always be provided. Travel reimbursement is provided for in-home visits. Board certification or fellowship training in palliative care or geriatrics is a plus. Must be comfortable working independently and in interdisciplinary teams. For additional information and to apply, please contact Nicholas Franzini, Physician Recruitment: [email protected] EOE M/F/D/VResponsibilities: - Provide comprehensive medical care to patients, including diagnosis, treatment, and follow-up care.- Conduct physical examinations and interpret medical history.- Order and interpret diagnostic tests, such as CT scans and medical imaging.- Perform procedures such as catheterization and dialysis.- Develop and implement treatment plans based on patient needs and medical standards.- Provide health coaching and education to patients to promote wellness and disease prevention.- Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care.- Monitor and manage patients in critical care or acute care settings.- Conduct esthetic laser treatments as needed.- Participate in quality improvement initiatives to maintain high standards of care. Skills: - Strong diagnostic evaluation skills to accurately assess patient conditions.- Proficiency in performing procedures such as catheterization and dialysis.- Knowledge of medical imaging techniques for diagnostic purposes.- Experience in critical care management for patients with complex medical conditions.- Ability to provide health coaching and education to promote patient wellness.- Familiarity with esthetic laser treatments for cosmetic purposes.- Understanding of medical standards and guidelines for delivering high-quality care.
