Provide personalized care to patients through medication therapy management services.
Conduct comprehensive medication reviews and identify potential drug therapy problems.
Collaborate with patients and healthcare providers to develop individualized treatment plans.
Educate patients on proper medication use, potential side effects, and potential drug interactions.
Monitor patient progress and make recommendations for adjustments to medication therapy as needed.
Maintain accurate and up-to-date patient records and documentation.
Ensure compliance with all state and federal laws and regulations related to pharmacy practice and medication therapy management.
Serve as a resource and provide guidance to other healthcare professionals on medication therapy management.
Stay current on new medications, treatment guidelines, and best practices in medication therapy management.
Demonstrate strong communication skills and develop relationships with patients to foster trust and promote medication adherence.
Contribute to the overall success of the pharmacy by meeting performance metrics and participating in team meetings and trainings.

Participate in continuous quality improvement initiatives to enhance the medication therapy management program.
