• *Responsibilities**

- Provide clinical pharmacy services focusing on population health and ambulatory care
- Manage and optimize medication therapy for patients with chronic conditions, particularly diabetes
- Collaborate with healthcare teams to support patient care plans and improve outcomes
- Perform traditional staffing duties as part of daily workflow
- Utilize Meditech electronic health records system for documentation and communication
- Participate in clinical program development and expansion initiatives
• *Qualifications / Desired Experience**

- Active PA pharmacist license
- Experience in ambulatory care or inpatient settings preferred
- Residency training is a significant plus
- Strong commitment to patient-centered care and collaborative practice

- Hybrid schedule with variable days/afternoons, primarily day shifts
- 8-hour shifts
